Taylor Swift Premieres Cinematic 'Patient Zero' Video at the VMAs, Starring Dakota Johnson and Colin Farrell

The self-directed short film casts Swift and Johnson as two women bonded by betrayal — both poisoned by the same man — shot by Oscar-winning cinematographer Emmanuel Lubezki.

The 2026 MTV Video Music Awards became a Taylor Swift takeover — with a side of Dakota Johnson and Colin Farrell. Swift debuted her latest cinematic feat, the music video for her single Patient Zero, during the September 27 awards show at the Peacock Theater in Los Angeles, and it is the darkest short film she has ever made.

As the 14-time Grammy winner concluded her acceptance speech for the inaugural MTV VMA Artist Director Honors, she announced the world premiere of her latest visual — "starring my friend Dakota Johnson and Colin Farrell," according to a transcript of her speech published by Just Jared. The two besties star as women who have dated the same man — Swift's ex, Johnson's current boyfriend — and by the final chorus, the video reveals he has been secretly poisoning both of them the whole time.

The song and the Encore

Patient Zero appears on The Life of a Showgirl: The Encore, the expanded edition of Swift's twelfth studio album that arrived Friday, September 25 with four additional tracks: Patient Zero, Cleveland!, Pink Clouding and Babylon. Three days after the Encore's release — and three days after Swift revealed the video's A-list cast — the visual made its world premiere at the September 27 VMAs, with the full video released to streaming platforms in the days that followed.

The song itself was inspired, Swift has said, by women comparing notes after realizing they had gone through similar experiences with the same man — and the official video brings that right out into the open, turning the comparison of notes into a gothic ghost story.

A gothic tale of two women and one poisoner

In the video, Swift and Johnson appear in matching gray trench coats, trousers, and black sweaters, with similar long hair and bangs, in a striking montage of mirrored shots that begins in a snow-covered graveyard. Johnson plays a woman trapped in a toxic relationship with Farrell's character — who controls, belittles, and gaslights her — while Swift emerges as a figure connected to his troubling past, a warning from before. The two twin in matching outfits as Swift "mentors" Johnson through the turbulence, which is made worse by Johnson feeling mysteriously ill.

Then comes the twist: it is revealed that Farrell has been secretly poisoning her, just as he did previously to Swift. The video ends with Swift visiting Johnson's grave — alongside newspaper clippings detailing past suspicions surrounding the mysterious deaths tied to Farrell's character. Patient Zero survived; the women who came after her were not as fortunate.

Cara Delevingne also appears as another woman connected to Farrell's character. In a post shared after the premiere — via Just Jared — Swift thanked her friend for agreeing to play what she jokingly described as the "lusty other woman."

Directed by Swift, shot by a legend

The video's cinematic pedigree is no accident. MTV and CBS had announced ahead of the ceremony that Swift directed the visual herself, with Academy Award-winning cinematographer Emmanuel Lubezki serving as director of photography. Lubezki's credits include The Revenant, Birdman, and Gravity — and in an Instagram post shared after the premiere — via Just Jared — Swift praised Lubezki, saying working with him was "something out of a dream sequence" and that she learned from watching him work.

She had high praise for her cast, too. Swift thanked Dakota and Colin for trusting her with the project, revealing — via Just Jared — that she and Dakota spent much of the shoot "twinning" and giggling together — and she singled out Farrell's performance as a villain as even more impressive because he is "an absolute light and as kind as they come" in real life.

In Swift's own words

After the premiere, Swift explained the cinematic recipe behind Patient Zero in a social-media post. Parade published her note, in which she wrote, "I got to explore the cinematic and literary genres I love so much," and said she wanted to blend "gothic ghost stories, true crime, psychological thrillers" in a setting unlike much of her previous work.

The stark house where Johnson's relationship with Farrell unravels was intentional, Swift said — she wrote that it served as "a metaphor for the coldness of a relationship that's faded and atrophied," according to Parade. As Swift sings about the song's subject, who "charmed" her family and friends while acting differently behind the scenes, the cinematic sequence goes silent as Johnson and Farrell walk into an award show and are confronted with the other woman, played by Delevingne. In one pointed moment, Johnson is seen stumbling and seemingly intoxicated, before leaving with Farrell and arguing in a car, during which, according to PEOPLE, Farrell tells her that she "loves" "humiliating [him] in front of everyone" and accuses her of lying about how many drinks she had.

The Artist Director Honors

The premiere was doubly fitting because of the award Swift had just accepted. The inaugural MTV VMA Artist Director Honors was created to recognize artists whose directorial work has advanced the art of music video and storytelling. Throughout her career, Swift has directed several of her own music videos, starting with "The Man" in 2019 — and in her acceptance speech she estimated that since her very first music video 20 years ago, she had starred in 60, 18 of which she wrote and directed.

Johnson herself introduced Swift at the ceremony before the singer accepted the honor. The night, in other words, was a showcase for both sides of Taylor Swift's artistry: the songwriter and the filmmaker. With Lubezki behind the camera, two of Hollywood's biggest actors in front of it, and a story that goes darker than anything she has attempted before, Patient Zero feels like a statement of intent — the 14-time Grammy winner is not done reinventing what a music video can be.
