Set in the dark fantasy world of Sanctuary, some fifty years after the fall of Malthael, Diablo IV returns to a land worn down by war and despair. The endless conflict between the High Heavens and the Burning Hells has left the forces of both angels and demons gravely weakened, and into this fragile void a long-banished evil stirs. Ages ago, the demon Lilith, daughter of the Prime Evil Mephisto, joined with the angel Inarius to create Sanctuary as a refuge from that eternal war, before her ruthlessness saw her cast into the void. Now a desperate cult performs a bloody ritual to summon her back into the world. As Lilith returns to reclaim her creation and reshape it in her image, a lone wanderer becomes entangled in her designs, drawn into a grim struggle against the growing darkness spreading across Sanctuary.
