Why this game matters

The champs are reeling, the Bills are rolling

A year ago, the New England Patriots won the AFC East and then the AFC, with Drake Maye finishing as the runner-up for league MVP. Three weeks into the encore, the Patriots are 1-2, their quarterback has been — in the words of one national columnist — "a complete tire fire," and now they walk into Highmark Stadium to face the 3-0 Buffalo Bills, who are laying nearly a touchdown in the first division game of the season for both teams.

The numbers explain the spread better than any film study. Buffalo is 3-0 and has allowed 78 points. New England is 1-2 and has scored 36 — an NFL-worst 12 points per game. The Bills' three wins: 36-31 at Houston, 41-31 over Detroit, 24-16 over the Chargers, a plus-23 differential built on an offense averaging more than 400 yards per game and ranking second in the NFL in total EPA. The Patriots' three games: a 10-13 loss at Seattle, a 20-3 win over Pittsburgh, and a 6-35 humiliation at Jacksonville. The Steelers win is the outlier. The Jacksonville loss is the ugliest single result on either side of this matchup.

Drake Maye's alarming regression

This is the story of the game, and there is no way around it. Drake Maye, the MVP runner-up, has completed just 63.7 percent of his passes for 585 yards, one touchdown, and six interceptions. He has not thrown a touchdown since Week 1. He has thrown multiple interceptions in three of his last four games, a stunning collapse for a quarterback who once went 20 straight weeks without a multi-pick game. His numbers are not just worse than last season — they are worse than his rookie year in 2024.

The Patriots rank 22nd in total offense, and the eye test is worse than the ranking. Maye looks rushed, his timing with his receivers is off, and the offense has not topped 20 points in a game all season. Now he gets the worst possible medicine: a road game in Orchard Park against a Buffalo defense coming off its best performance of the season against the Chargers, in front of a crowd that smells blood in a division rivalry. If Maye cannot flip the switch on Sunday, the conversation in New England shifts from "slow start" to something much darker — because franchise quarterbacks coming off MVP-caliber seasons do not get long leashes when they look like this.

Josh Allen, the MVP favorite, and a historic ground game

On the other sideline, everything is going right. Josh Allen is the overwhelming favorite in the MVP odds, and the Bills' offense is a machine: No. 1 in the NFL in EPA per rush attempt, over 400 yards per game, and 3-0 against the spread. James Cook III has been the engine, ripping off chunk plays behind an offensive line that is moving people — the Reuters photo of Cook diving into the end zone has become the defining image of Buffalo's September.

What makes the Bills genuinely scary is that they have not needed their defense to be great. Buffalo has allowed 78 points and has not won a game by making it boring — the 36-31 and 41-31 shootouts prove this team can win any kind of game. Against a New England offense averaging 12 points, the Bills do not need a shootout. They need competence, and they have been nothing but competent. This is Buffalo's third consecutive home game, the crowd will be feral for a division statement, and the Bills know a win here plus a Patriots loss drops New England to 1-3 and effectively buries the defending division champs before Columbus Day.

New England's defense: the one reason for hope

If the Patriots are going to steal this game, it will be because of the side of the ball nobody is talking about. New England's passing defense has been one of the best in the league, with Christian Gonzalez and Carlton Davis locking down the skies, and the front seven has been a full team effort — Elijah Ponder and Gabe Jacas standing out up front, with Christian Elliss arguably the team's best all-around defender in both coverage and against the run.

The blueprint exists. The Patriots won in Buffalo last year, snapping the Bills' four-game home winning streak in the series, and the road team won and covered both meetings a season ago. Divisional familiarity is real: New England has seen these fronts and these coverages twice a year, and that knowledge is a genuine drag on favorites. The problem is the talent gap — as one preview noted, familiarity gets overrated when one team simply has much better players. The Patriots' defense can keep this close for a half. But 60 minutes against Allen, Cook, and this offensive line is a different ask, especially when your own offense keeps going three-and-out and putting you back on the field.

A rivalry game with a season inside it

Strip away the records and this is still Patriots-Bills, still the rivalry that has defined the AFC East for a generation. Buffalo has dominated the recent series at 9-3 over the last six years, but New England's win in Orchard Park last season proved the building holds no fear for this roster. The over is 7-3 in the last 10 meetings — these teams tend to score on each other, which is worth remembering with a total sitting near 49.

The stakes could not be clearer. A Bills win makes them 4-0, alone in first place, with the defending champs buried two games back and a head-to-head tiebreaker in hand. A Patriots upset — and it would be a genuine upset — makes New England 2-2 with the tiebreaker flipped and the entire "Maye is broken" narrative rewritten in one afternoon. Everything about the matchup says Buffalo: the offense, the defense's trajectory, the home field, the quarterback play. But rivalry games have a way of ignoring everything the matchup says. The Patriots' season might depend on it.
