Karen Kilgariff Hosts Heart, Apple's New True-Crime Podcast About a Transplant, a Widow and Two Deaths

The eight-episode Apple Original Podcast arrives on Apple Podcasts on October 19, following two families linked by one heart — and by two deaths that were both officially concluded to be suicides.

Apple's next true-crime podcast has a premise that sounds, at first, like a love story. A man receives a lifesaving heart transplant. He falls in love with his donor's widow. Local newspapers, according to Apple, presented what happened next as a miraculous romance. Then the story turns. The recipient, Sonny Graham, dies the same way as his heart donor, Terry Cottle — and in both cases, according to Apple, the official conclusion is suicide.

That is the story at the centre of Heart, an eight-episode Apple Original Podcast hosted by Karen Kilgariff, the co-host of My Favorite Murder. The Mac Observer reported on October 17 that the series arrives on Apple Podcasts on October 19, 2026, after Apple TV announced it with a trailer on October 12. iClarified, reporting Apple's preview on October 12, independently described the same eight-episode series, the same October 19 premiere and the same story of two families connected by a heart transplant. Listeners who follow true crime in Previously's podcast library will recognise why Apple chose this host — and why this particular story needs careful handling rather than sensation.

How the release works

Apple is splitting the release in two, according to The Mac Observer's reporting of Apple's announcement. Apple TV subscribers can hear every episode on launch day, October 19 — all eight at once, with nothing left to wait for. Everyone else can still hear the whole series for free on Apple Podcasts, but on a slower schedule: the first three episodes are free on launch day, followed by one new episode each week through the finale on November 23, 2026.

iClarified's report of Apple's preview matches that structure exactly: all eight episodes on October 19 for Apple TV subscribers, the first three episodes that day for everyone else, and one new episode weekly through November 23. The Mac Observer also noted Apple's listed price for Apple TV — $14.99 per month, with a seven-day free trial for new subscribers, according to Apple — and that listeners can follow the show in Apple Podcasts now ahead of the premiere. That two-track release has become a familiar Apple pattern: subscribers get the binge, everyone else gets the weekly wait, and the conversation around a series stretches across more than a month instead of a single weekend.

Two men, one heart

According to Apple, as reported by both The Mac Observer and iClarified, Heart centres on Sonny Graham, who received a heart from Terry Cottle in a lifesaving transplant, and who later fell in love with Cottle's widow. That relationship is the part of the story that local newspapers presented as miraculous — a second chance at life that seemed to extend, almost impossibly, into a second chance at love for two people brought together by loss.

Apple's synopsis then takes its tragic turn. Sonny Graham dies the same way as his donor. In both men's cases, the official conclusion is suicide. Those are the established facts as Apple presents them, and Heart, on Apple's own description, does not pretend to overturn them by assertion. Instead, the series follows what happened around them: friends and family recall changes in Sonny's behaviour after the transplant, and Apple says some question whether the heart he received could have carried traces of the donor's trauma.

The show's page on Apple's press site puts its central question bluntly, according to Apple, in the wording both reports carry: “Are the two men's deaths just a coincidence, or are there darker forces at work?” That is Apple's framing — a question the series sets out to explore — not a conclusion the reporting establishes. Previously is presenting it the same way: as the question the podcast asks, about two real families, two real deaths, and a connection neither family chose.

Three years of reporting behind eight episodes

Apple says the series draws on three years of reporting, according to both The Mac Observer and iClarified, and follows the families' search for answers through court filings, public records and personal accounts. It examines the connections between the two men's lives and deaths, with Apple listing “trauma, identity and the mysteries of the heart” as its themes, according to Apple.

That sourcing matters for a story like this one. A transplant, a widow, a romance and two deaths could easily be told as a curiosity — the kind of tale that travels because it is strange. Three years of reporting, court filings and public records point the other way: toward a documented account in which the families themselves, and what they remember, sit at the centre. The personal accounts Apple cites include the recollections of friends and family who noticed changes in Sonny after the transplant — the human detail on which the series' central question rests — set against the paper record of filings and documents.

Who is making it

Heart comes from Plan B, the production company led by Brad Pitt, Dede Gardner and Jeremy Kleiner, together with Blanchard House, the British audio studio. The Mac Observer noted that Apple calls Heart the latest collaboration between Apple and Plan B, a partnership that already includes the Brad Pitt racing feature F1 The Movie, with an upcoming father-son dramedy film starring Mark Wahlberg and the feature The Waffle House Index also on the way, according to Apple. Blanchard House, for its part, has made Apple Original Podcasts before: its earlier work for Apple includes Extrasensory, The Pirate of Prague and Adrift.

And then there is the host. Kilgariff is a writer, producer, podcast host and comedian, best known for co-hosting My Favorite Murder. In Apple's announcement, she said: “Working with such a talented production team has made hosting ‘Heart’ an absolute dream. I'm so proud to be a part of this show.” For listeners who know her from My Favorite Murder, the fit is plain: a host long practised at telling true-crime stories in a human voice, now fronting a reported series from two established production houses rather than a two-microphone conversation. Where it fits, and when to listen

Heart joins a growing shelf of Apple Original Podcasts that Apple says are now streaming on Apple Podcasts, among them Unicorn Girl, Easy Money: The Charles Ponzi Story, Adrift, the duPont-Columbia Award winner The Line, the Webby Award nominee Hooked, Run, Bambi, Run, Operation: Tradebom, My Divo, The Pirate of Prague, Magnificent Jerk and Missed Fortune, alongside official companion podcasts for the Apple TV series Foundation and For All Mankind and for the docuseries The Big Conn. It is, in other words, a flagship slot — and Apple is treating the release like one, with the full-season drop reserved for its own subscribers.

The premiere is Monday, October 19, 2026. Subscribers can hear the whole investigation at once; everyone else gets three episodes on day one and a weekly appointment through November 23. Either way, this is a story worth approaching the way its makers, on Apple's account, approached it: through the records, the families and the questions they are still asking — not through the miracle headline the newspapers first gave it.
