ABC's medical institution returns Thursday at 10 p.m. ET with a Sea-Tac collision, a controversial VIP floor — and its first season without Owen Hunt and Teddy Altman as regulars.
Twenty-three seasons in, Grey's Anatomy still opens the way it trusts most: a disaster, a full trauma bay and a hospital arguing with itself. Season 23 premieres tonight, Thursday, October 15, at 10 p.m. ET on ABC — streaming next day on Hulu — with "Collide," a premiere that sends a wave of critically injured patients into Grey Sloan Memorial at the exact moment the hospital is unveiling its most controversial addition in years.
Inside "Collide": A Crash Meets a Ribbon-Cutting
ABC's official description of the premiere is blunt about the collision course: after a collision at Seattle-Tacoma International Airport sends a surge of critically injured patients to Grey Sloan, the unveiling of its controversial new VIP floor is thrown into chaos as doctors race to save lives amid impossible choices. The episode is directed by Debbie Allen — who also recurs as Dr. Catherine Fox — and written by showrunner Meg Marinis with Jess Righthand, per the season's episode records.
The VIP floor is not set dressing; it is the season's argument. The new wing is headed by Dr. Seth Norton, played by James Wolk in a recurring role, a surgeon-turned-businessman who runs Olympic Regent Health, a luxury-care operation where patients pay out of pocket for elevated care. Marinis has teased that Norton arrives ready to ruffle feathers, and premiere first-look coverage shows Catherine Fox personally introducing him to Meredith Grey — who, characteristically, is not interested in the tour. Wolk is set to appear in at least four episodes, per Parade's premiere reporting, which makes Norton a season presence rather than a premiere-week gimmick: the man selling a two-tier hospital, installed inside the hospital that has always insisted care comes first.
The First Season Without Owen and Teddy
The bigger change is who is not in the building. Season 23 is the first season without Kevin McKidd and Kim Raver as series regulars — McKidd had been a regular since Season 5, Raver most recently since Season 15, per the season's production records. Their exits were written into the Season 22 finale: Owen Hunt survived the bridge collapse that closed last season, reconciled with Teddy Altman, and the two left Seattle with their children for Paris. Both departures were announced in March 2026, when ABC renewed the series — and the door has been publicly left open, with McKidd already back behind the camera directing the season's second episode, "Slippin' and Slidin'," airing October 22.
Losing two regulars of that tenure at once would stagger most dramas. The Grey's argument, made by cast members in the renewal coverage, is that the structure absorbs it: the hospital is the main character, and the ensemble refills. Caterina Scorsone, who plays Amelia Shepherd, told Entertainment Weekly in September that the show's structure — creator Shonda Rhimes' original build — is what gives it that resilience.
Meredith Is Here — on Meredith's Terms
Ellen Pompeo appears in the premiere and continues in the role she has held since scaling back in Season 19: Meredith Grey as a limited-capacity presence, the series' narrator and an executive producer, per PEOPLE's season preview. The Season 22 finale set her up with an engagement to Nick Marsh (Scott Speedman, recurring this season) after his near-fatal injuries in the bridge collapse — a storyline the new season inherits, with Meredith pulled between the Boston life she has been building and a Grey Sloan that keeps finding reasons to need her. Tonight she is in the building for the VIP floor unveiling and the crash intake alike, which tells you how the show still routes its biggest nights: through her.
Around her, the returning ensemble is deep: Chandra Wilson as Miranda Bailey and James Pickens Jr. as Richard Webber — with Pompeo, the three originals who have been with the series for its entire run — plus Scorsone's Amelia, Camilla Luddington as Jo Wilson, Jason George as Ben Warren, Chris Carmack as Link, Anthony Hill as Winston Ndugu, Alexis Floyd as Simone Griffith, Harry Shum Jr. as Benson "Blue" Kwan, Adelaide Kane as Jules Millin, Niko Terho as Lucas Adams and Trevor Jackson as Wes Bryant. Jennifer Landon recurs as Dr. Toni Wright, and Sarah Levy joins as Corrie.
The Season Ahead
Marinis is in her fourth year as showrunner, and the season is set to run 13 episodes, per Disney's season announcement reporting. The early calendar is already public: "Collide" tonight, "Slippin' and Slidin'" on October 22 and "It's All Wrong, but It's All Right" — also directed by Allen — on October 29, with new episodes weekly on Thursdays at 10 p.m. ET on ABC and next-day streaming on Hulu, where the full 22-season catalog also lives. ABC has not announced a finale date.
What Season 23 has to prove is not that Grey's Anatomy can survive change — two decades of cast turnover settled that — but that it can still argue about something. A luxury floor inside a public-facing hospital, a businessman-surgeon selling access, and a mass-casualty night that does not care what anyone paid: that is a thesis, not a gimmick. "Collide" premieres tonight at 10 p.m. ET on ABC.
